Your current coverage will end at midnight on your last day of work.

You have the option of continuing your current medical, dental and vision plan, as well as Flexible Spending Account participation and access to the Employee Assistance Program, all through COBRA, for up to 18 months.
  • To continue your medical, dental and vision plans, the cost will be 102% of the monthly premium, which includes the full premium cost along with an administrative fee.
  • If you wish to continue coverage through COBRA, you must enroll within 60 days of your end of coverage.
  • If you do not wish to continue coverage through COBRA, you can contact Mylo (through Lockton) to learn about your health care exchange options. Mylo can also assist you in finding an affordable health plan from over 40 different carriers, including Medicare supplemental plans. Note, you have 60 days from leaving Lennox to enroll in the health care exchange.

Coverage ends on your last day of work.
  • If you would like to convert your life insurance, please request a Life Conversion Information Request form from your HR representative.
  • If you are enrolled in Critical Illness or Accident Insurance, VOYA will contact you with information on how to convert coverage.
  • Coverage and costs may not be the same as the group coverage available as an Lennox employee.
  • If you are enrolled in the Legal Services Plan, LegalShield will mail you information on how to port your coverage.
  • You have 60 days to port from your date of termination. The price and plan are the same.
Disability coverage ends on your last day worked.
  • Participation in the Health Care Spending Account and Dependent Care Spending Account plans end as of your date of termination.
  • Expenses incurred after your date of termination are not eligible for reimbursement.
  • If you have outstanding expenses incurred prior to your date of termination, you have until March 31 of the following year to submit your claim for reimbursement.
Accrued, unused vacation time prior to your last day worked will be paid out according to the accrual schedule. You will not receive payment for unused holiday or sick time.
  • You may have several options available with your 401(k) account depending on the balance. Visit the Fidelity website or contact Fidelity’s Customer Service Team.
  • If you have a loan against your 401(k), please contact Fidelity for repayment options and to understand any potential tax implications.
