If you (and your spouse) are enrolled in the Lennox medical plan, you are eligible to complete a free annual health screening through the LIIveWell program.
You can avoid paying the $75 per person per month Wellness surcharge in the following plan year by maintaining or improving your physical health year over year.
NOTE: There is a separate $75 per month Wellness surcharge for your covered spouse.
If you are not enrolled in the Lennox medical plan, you do not need to complete a health screening.

What is a Health Screening?
A health screening evaluates your overall health and provides a confidential assessment that can benefit you in the long-term. With this information, you can catch potential health risks early on and take action or seek treatment before symptoms appear.You and your covered spouse are eligible to complete a free annual health screening in the current plan year if you both enrolled in Lennox’s medical plan before August 1.

- You, and your covered spouse, must each complete and submit your annual health screening results in LIIveWell by September 30, 2024.*
- New hires, and their covered spouses, must each complete and submit their initial health screening results in LIIveWell during the first year of employment to set a baseline.*
* If you and/or your spouse are unable to complete the requirements due to a medical condition, your physician can fill and sign the Medical Waiver form. The completed form(s) can be submitted via email, fax, or directly uploaded into your LIIveWell account by November 29, 2024.
* If you or your spouse are pregnant, you do NOT need to complete a health screening. Instead, you must complete and submit the Pregnancy Waiver form via email, fax, or directly uploaded into your LIIveWell account by November 29, 2024.

- Onsite Health Screening
Visit LIIveWell to schedule an appointment. Ask your local HR representative for dates and times. - LabCorp Facility
Visit LIIveWell to find a LabCorp near you that offers “Employee Wellness with Body Measurements.” Download the LabCorp Voucher and take it with you. The lab will submit your results automatically. - Visit Your Doctor
Print the Physician Form on LIIveWell and bring it with you to your in-person doctor’s appointment. Ask your doctor to complete the form during your exam. Either you or your doctor can submit the form via email or fax. You can also directly upload the form into your LIIveWell account.
