Start by taking a quick Well-Being Questionnaire about your daily lifestyle, physical activity, and health goals.
LIIveWell will customize your experience according to your needs and interests. The focus is on YOU!
- Design your own wellness plan focused on fitness, nutrition, sleep, mental health, etc.
- Set daily targets and track your long-term progress.
- Stay motivated and challenge yourself with individual tasks.
- Access 1-on-1 coaching with a certified WebMD wellness expert.
- Have fun by participating in year-round group challenges.
- Find quick and convenient healthy recipes.
All LIIveWell participants can access FREE health coaching and mental health coaching.
Meet with a certified life coach to create a daily action plan based on your physical and mental health goals. Some common goals you can aim for include:
- Enhancing your daily routines
- Increasing your energy levels
- Adding more steps in your day
- Organizing easy weekly meal planning ideas
- Improving your diet (i.e., adding more protein, reducing carbs, etc.)
- Making time for self-care
- Prioritizing 10 mins of quiet time each day
- Getting adequate sleep
- Reducing screen time
- And more!

LIIveWell Annual Health Screening
Employees and spouses enrolled in a Lennox medical plan must complete a FREE annual health screening to avoid the per person monthly $75 Wellness surcharge in the next plan year.
Annual health screenings take a short amount of time and can be completed each plan year starting January 1.
First time participants must register on LIIveWell.

NOTE: Health screenings are not required if you and/or your legal spouse are not enrolled in the medical plan.
This FREE annual screening benefit helps you gain insight into your long-term health.
What is a health screening?
A health screening is a 15 – 20 minute confidential assessment that measures 5 key metrics of physical health and enables you to:
- Identify potential risks early,
- Take action before symptoms appear, and
- Make informed decisions about your health.
Both you and your covered spouse must meet or improve 3 of 5 health screening metrics to avoid the per person monthly $75 Wellness surcharge in the next plan year.
Employees (and covered spouses) can complete an annual health screening each plan year starting January 1.
New hires (and spouses) who enroll in the medical plan before August 1 of the current plan year also qualify.
To avoid the $75 Wellness surcharge in 2027…
- You (and your covered spouse) must each complete and submit your annual health screening results in LIIveWell to confirm 3 of 5 measured health metrics have improved or have remained within the healthy target range by September 30, 2026.*
- New hires (and your covered spouse) are each required to complete an annual health screening during the first year of employment to set a baseline. Baseline health metrics are not required to be in the healthy target range. Submit your screening results in LIIveWell by September 30, 2026.*

Where should you go?
LIIveWell offers three convenient and confidential ways to complete your health screenings.*
- Visit Your Doctor
Schedule an appointment with your doctor and bring the printed Physician Screening Form with you. Your doctor must complete and sign the form using results from an office visit between January 1 and September 30. Please upload the signed form to your LIIveWell account for review and approval.- If your spouse is participating, he/she should follow the same steps.
- LabCorp Facility
Visit LabCorp.com to locate and a schedule an appointment at a location offering “Employee Wellness with Body Measurements.” Print the Lab Requisition Form and bring it with you. The form must completed and signed using results from a lab visit between January 1 and September 30. LabCorp will automatically submit your screening results to LIIveWell for approval.- If your spouse is participating, he/she should follow the same steps.
- On-Site Health Screening
Lennox coordinates on-site health screening events with licensed medical providers. On-site screenings are open to both employees and covered spouses. Your results will be automatically submitted to LIIveWell for review. Contact your local HR team for event dates/times.

How should you prepare for your screening?
For the most accurate results, avoid eating and drinking (except water) for at least 9 hours before testing.
- Be sure to talk to your doctor before fasting if you are diabetic.
- Take your regular medications.
- Avoid strenuous exercise and alcoholic beverages for 24 hours.
- Drink plenty of water the day before and on the day of your appointment.
What is the expected wait time for screening results?
It can take up to 10 days for your confidential health screening results to appear in your LIIveWell account.
If you have questions or need an approval status, call (844) 800-2454.
LIIveWell will update your status when you (and/or your spouse) achieve 0%, 50%, or 100% task completion.
- 0% – Health screening not completed.
- 50% – Health screening completed but did not pass because your results are not within the target range.
- 100% – Health Screening completed, you achieved meeting 3 of 5 health metrics.
