Lennox’s Critical Illness Insurance plan automatically includes a $100 Wellness Benefit payment that can be claimed after you and your covered spouse/children complete at least one eligible health screening test in the plan year.

The $100 Wellness Benefit is included with your Critical Illness insurance plan at no additional cost.

  • Your annual benefit amount is $100.
  • Your covered spouse’s annual benefit amount is $100.
  • Each covered child’s annual benefit amount is $100.

It’s easy to claim your $100 Wellness Benefit.*

  1. Visit the Voya Online Claims Center at Voya.com/Claims.
  2. Click “Get Started” under “Start a Claim.”
  3. Click on “Let’s Get Started” then select “Policy/ Employee.”
  4. Select who the claim is for and continue.
  5. Fill in your personal information and enter the Lennox group number: 67102-9.
  6. Select “Wellness/Health Screening”, then “Critical Illness.”
  7. Click “Submit.”

If your claim is approved, you should receive your paid benefit within 10 business days of the approval.

*The Wellness Benefit is payable up to a maximum of one time per covered person per calendar year. Review the Certificate of Coverage for a full list of eligible health screening tests.
