We have partnered with WebMD to launch LIIveWell, a free online wellness program for you and your family.

Complete a brief Well-Being Questionnaire to assess your overall well-being, health habits, physical activity, emotional resilience, and more. This is the first step to making the most of this self-guided program.

WebMD offers a FREE wellness and mental health coaching benefit to all LIIveWell participants.

Certified expert coaches can help you create and maintain a daily action plan that meets your physical and mental needs. Some common goals you can aim for include:
  • Enhancing your daily routines
  • Increasing your energy levels
  • Adding more steps in your day
  • Organizing easy weekly meal planning ideas
  • Improving your diet (i.e., adding more protein, reducing carbs, etc.)
  • Making time for self-care

LIIveWell Annual Health Screening

Employees and spouses currently enrolled in a Lennox medical plan must complete a FREE annual health screening to avoid the monthly $75 Wellness surcharge(s) in the next plan year.

Annual health screenings are confidential, take a short amount of time, and can be completed each plan year starting January 1. Results can be easily submitted directly in your LIIveWell account.

NOTE: Health screenings are not required if you and/or your legal spouse are not enrolled in the medical plan.

Our FREE annual screening benefit helps you gain insight into your health and avoid the monthly $75 Wellness surcharge(s) in the next plan year.*

To qualify for the Wellness surcharge waiver, your screening results (and those of your covered spouse) must indicate an improvement or remain within a healthy target range over time.

What is a health screening?

A health screening is a 15 – 20 minute confidential assessment of your physical health, lifestyle, and well-being. This annual check-up will help you identify potential health risks early, take action before symptoms appear, and make informed decisions about your health.

* There is a separate additional $75 per month Wellness surcharge for your covered legal spouse.


Employees (and covered spouses) can complete an annual health screening each plan year starting January 1.

New hires (and spouses) who enrolled in the medical plan before August 1 of the current plan year also qualify.

To avoid the Wellness surcharge(s) in 2026…

  • You, and your covered spouse, must each complete and submit your annual health screening results in LIIveWell to confirm 3 of 5 measured health metrics have improved or have remained within the healthy target range by September 30, 2025.*
  • New hires, and their covered spouses, are each required to complete an annual health screening during the first year of employment to set a baseline. Baseline health metrics are not required to be in the healthy target range. Submit your screening results in LIIveWell by September 30, 2025.*
* If you and/or your covered legal spouse are unable to complete the requirements due to a medical condition, your physician can fill and sign the Medical Waiver form. The completed form(s) can be submitted via email, fax, or directly uploaded into your LIIveWell account by November 28, 2025.

* If you or your spouse are pregnant, you do NOT need to complete a health screening. Instead, you must complete and submit the Pregnancy Waiver form via email, fax, or directly uploaded into your LIIveWell account by November 28, 2025.

LIIveWell’s annual screening measures the 5 key health metrics below against your applicable target goals.

Both you and your covered spouse must meet or improve 3 out of 5 health screening metrics to avoid the $75 per person per month Wellness surcharge in the following plan year.

Ready to schedule your screening?


LIIveWell offers three convenient and confidential ways to complete your health screenings. You can also inquire about alternative options by calling (844) 800-2454.
  • Onsite Health Screening
    Lennox coordinates onsite health screening events with licensed medical providers. Check your LIIveWell account or contact your local HR representative for dates/times. The screening appointment takes 15 – 20 minutes, and your results are automatically submitted.
  • LabCorp Facility
    Visit your LIIveWell account to locate a nearby LabCorp that offers “Employee Wellness with Body Measurements.” Download the Lab Requisition Form and present it at the lab. LabCorp will automatically submit your screening results.
  • Visit Your Doctor
    Print the Physician Form on LIIveWell and bring it with you to your in-person doctor’s appointment. Ask your doctor to complete the form during your exam. You can upload your signed Physician Form in your LIIveWell account (preferred method). Alternatively, you or your doctor can submit the form by fax or mail. Late forms will not be accepted.
How should you prepare for your screening?
For the most accurate results, avoid eating and drinking (except water) for at least 9 hours before testing.
  • Be sure to talk to your doctor before fasting if you are diabetic.
  • Take your regular medications.
  • Avoid strenuous exercise and alcoholic beverages for 24 hours.
  • Drink plenty of water the day before and on the day of your appointment.
What is the expected wait time for screening results?
It can take up to 10 days for your confidential health screening results to appear in your LIIveWell account. If you experience any issues, please call (844) 800-2454. You can also view your previous year’s health data in your account by clicking Health Record > Health Goals.

NOTE: First time participants will need to create a free LIIveWell account.

Track your progress towards meeting your annual health screening requirements.

LIIveWell will update your status when you (and/or your spouse) achieve 0%, 50%, or 100% task completion.

  • 0% – Health screening not completed.
  • 50% – Health screening completed but did not pass because your results are not within the target range.
  • 100% – Health Screening completed, you achieved meeting 3 of 5 health metrics.

Find and click the “Completed” tab to view information about your completed health screening tasks and results for each measured metric. You can also see and compare your previous screening results.
