*A qualifying medical illness diagnosis must occur after the effective date of your Critical Illness Insurance policy to be eligible for the benefit payment. Review the Certificate of Coverage for more details.
The Critical Illness Insurance benefit is voluntary and can only be purchased for yourself, your spouse, and/or your dependent children when you are initially eligible or during the annual Open Enrollment period.

Your policy automatically includes a $100 Wellness Benefit. Each covered person who completes at least one health-related screening qualifies for an annual one-time payout.
Common eligible screenings include a routine physical, eye exam, dental exam, pap smear, mammogram, colonoscopy, and many more. Lennox’s LIIveWell Annual Health Screening also qualifies!

With your policy…
- Your annual benefit amount is $100.
- Your covered spouse’s annual benefit amount is $100.
- Each covered child’s annual benefit amount is $100.

* The $100 Wellness Benefit is payable up to a maximum of one time per covered person per calendar year. Review the Certificate of Coverage for a full list of eligible health screening tests.

- Visit the Voya Online Claims Center at Voya.com/Claims.
- Click “Get Started” under “Start a Claim.”
- Click on “Let’s Get Started” then select “Policy/ Employee.”
- Select who the claim is for and continue.
- Fill in your personal information and enter the Lennox group number: 67102-9.
- Select “Wellness/Health Screening”, then “Critical Illness.”
- Click “Submit.”

- Select the benefit you would like to file a claim for. Click “Get Started.”
- Review the information to have on hand. Click “Let’s Get Started.”
- Select “Policy/Employee.”
- Fill in your personal information and enter Lennox group #67102-9.
- Upload required supporting documentation.
- Electronically sign and click “Submit.”
- Check your email for your submission confirmation number.
