Chronic Condition Support Program

With Quantum Health’s Chronic Condition Support Program, nurses are always on call and available 24/7 to help you manage these chronic conditions: asthma, diabetes, chronic obstructive pulmonary disease (COPD), coronary artery disease (CAD), or congestive heart failure (CHF). You may qualify for $0 condition-specific medications!

$0 Copay Prescription Program

Through Quantum Health’s $0 Copay Prescription Program, medical plan members diagnosed with one of the chronic conditions listed above can receive specific approved medications free of charge (no copay required). NOTE: If you are currently participating or want to qualify in 2026, you must complete 100% of condition-specific health management activities by the end of 2025.

Tobacco Cessation Program

Quantum Health offers support to help you and/or your spouse quit using tobacco and/or smoking.

Program benefits include:
  • FREE nicotine replacement aids (patches, gum, lozenges, etc.)
  • FREE 1-on-1 quit counseling sessions (choose your own coach via convenient scheduling)
Remember: If you complete 5 quit counseling sessions by the current year’s deadline, you’ll stop paying the per person monthly Tobacco Use surcharge(s) AND you’ll get a refund of the amount already paid.
