With Quantum Health’s Chronic Condition Support Program, registered nurses are on call 24/7 to help you manage these chronic conditions: asthma, diabetes, chronic obstructive pulmonary disease (COPD), coronary artery disease (CAD), or congestive heart failure (CHF). You may even qualify for $0 condition-specific medications!$0 Copay Prescription Program
Lennox medical plan members diagnosed with one of the chronic conditions listed above may qualify for Quantum Health’s $0 Copay Prescription Program, which covers certain approved medications at no cost (no copay required). NOTE: You must complete 100% of the required care path activities to qualify for this benefit.Tobacco Cessation Program
Quantum Health has a dedicated program in place to help you and/or your spouse quit smoking.Program benefits include:
- FREE nicotine replacement aids (patches, gum, lozenges, etc.)
- 5 quit counseling sessions for each person – choose your own coach and schedule.
