Your Next Steps
The following steps should be taken in case your dependent was covered by a Lennox benefit plan:
- Within 31 days, visit BenefitSource or call (800) 284-4549 to report your new life event.
- Main menu > Life Event > All Other Life Events
- Scroll down and click on ‘Other Life Events’
- Click on the applicable event in the drop down – ‘Spouse Death’ or ‘Child Death’
- Within 31 days, review your existing benefit elections in BenefitSource to determine what changes are required. Changes in coverage can be requested directly through your account.
- Within 60 days, upload proof of life event (a death certificate) in BenefitSource for review. To check the status, please call (800) 284-4549. New coverage changes will not take effect until approved.
- Review your Company-paid Life and AD&D Insurance beneficiaries. Call (877) 236-7564 for help.
- Review your Supplemental Life Insurance policies and beneficiaries. Call (877) 236-7564 for help.
- Review the beneficiaries on your 401(k) Retirement Plan by visiting Fidelity NetBenefits.
- If participating in the FSAs, consider adjusting the pre-tax amount(s) you contribute.
- Review your paid time off benefits and take the time you need to cope.
- Contact a Quantum Health Care Coordinator for guidance on your medical benefits.
- Connect with the 24/7 Lennox EAP to access free 1-on-1 counseling support.
