Your Critical Illness Insurance policy automatically includes a $100 Wellness Benefit.

Each covered person who completes at least one health-related screening qualifies for the payment. Common eligible screenings include:

• Lennox’s annual health screening,
• routine physicals,
• eye exams,
• dental exams,
• pap smears,
• mammograms,
• colonoscopy,
• and many more!

With your policy…

• Your annual benefit amount is $100.
• Your covered spouse’s annual benefit amount is $100.
• Each covered child’s annual benefit amount is $100.

It’s easy to claim your $100 Wellness Benefit.

1. Visit the Voya Online Claims Center at Voya.com/Claims.
2. Click on “Get Started” under “Start a Claim”. Enter Lennox group #67102-9 and your policy number.
3. Complete the questions regarding the health screening test and submit your Wellness Benefit claim (one person per claim). Save your confirmation number.
4. Receive a benefit payment for the covered individual for whom an eligible claim was filed.

NOTE: The CII Wellness Benefit is payable up to a maximum of one time per covered person per calendar year. Review the Certificate of Coverage for a full list of eligible health screening tests.
