You can purchase coverage for yourself, your spouse, and/or your dependent children during Open Enrollment.
A $100 Wellness Benefit is also available to each covered person who completes an eligible health screening test

With your policy…
- Your annual benefit amount is $100.
- Your covered spouse’s annual benefit amount is $100.
- Each covered child’s annual benefit amount is $100.
It’s easy to claim your $100 Wellness Benefit.
- Visit the Voya Online Claims Center at Voya.com/Claims.
- Click on “Get Started” under “Start a Claim”. Enter Lennox group #67102-9 and your policy number.
- Complete the questions regarding the health screening test and submit your Wellness Benefit claim (one person per claim). Save your confirmation number.
- Receive a benefit payment for the covered individual for whom an eligible claim was filed.
- Visit the Voya Online Claims Center at Voya.com/Claims.
- Click on “Get Started” under “Start a Claim”. Enter Lennox group #67102-9 and your policy number.
- After answering a few questions, you will be asked to upload supporting documentation for your claim.
- Electronically sign and submit your claim. You will immediately receive an email with a confirmation number letting you know the claim submission was successful.
Keep in Mind
Your Critical Illness Insurance policy premium is paid with after-tax dollars via automatic payroll deductions.
